Prophylactic Surgical Extractions of Impacted Third Molars in Young Patients : Journal Review

Abstract
Purpose: The surgical extraction of impacted third molars (ITMs) is a very common dental surgical procedure. However, there are still controversies that asymptomatic ITMs have to be removed prophylactically or not. The aim of the study was to investigate the pros and cons of the prophylactic removal of ITMs in young patients through current journal review.

Materials and methods: A literary search was performed in Pubmed and ScienceDirect.

Results: A total of 41 studies were included in the review. To prevent distal caries, periodontitis, and external root resorption of adjacent second molar, prophylactic removal of ITMs was appropriate. Also, for the better treatment result of orthodontic treatment and orthognathic surgery, prophylactic removal of ITMs was also appropriate. However the presence of ITMs is associated with increased risk of mandibular condyle fracture and simultaneously decreased risk of mandibular angle fracture. The socioeconomic aspect of prophylactic removal of ITMs has to be reconsidered in the patients¡¯ own society.

Conclusion: The dental clinicians have to plan the wise evidence-based treatment decision for asymptomatic ITMs in young patients.
